Reporting a claim
This form tells Cusick Insurance Brokers about your claim so we can advise you. It does not satisfy the notice requirements in your policy.
Your policy specifies how and to whom a claim must be reported. Notice to us is not notice to your insurer.
You are responsible for reporting the claim directly to your insurer in the manner your policy requires, and for getting written confirmation from the insurer that it was received.
We can help you report it. Call us if you want that help. Reporting deadlines can be strict and late notice may affect a claim, so don’t rely on this form alone to meet one.
